THE HSA PLAN IS MADE UP OF 2 PARTS:
HEALTH SAVINGS ACCOUNT:
 You make a monthly contribution for up to the annual amount of your health insurance deductible. You will receive a MasterCard Debit Card (tied to your Health Savings Account) to use to pay any medical expenses you incur, until you have satisfied your health insurance deductible.
You may even use your MasterCard Debit Card to pay for traditionally non-covered expenses such as dental (including braces), vision exams, eyeglasses, maternity, chiropractic, etc.
Whatever you don't use, YOU KEEP!
Your contributions to your HSA Savings Account are tax-deferred to age 65. In other words, this works like a Medical IRA!

All covered doctor visits, prescriptions, lab work, etc., goes toward meeting One Deductible for all insureds. Choice of either $5100 or $9900 annual total deductible for entire family ($4300 or $3500 deductibles are also available for family). OR... Choice of either $2550 or $4950 annual total deductible for individual ($2150 or $1750 deductibles are also available for individual).
100% coverage up to Lifetime Maximum after annual deductible is met. Total out-of-pocket maximum exposure per year is only the amount of your deductible (which is funded by your
